We have an arrangement with local pharmacies to deliver your medication to you at home. Your GP will still review your medication each time and review your condition as before. This will save you time and multiple trips to the GP and chemist. To facilitate this service, fax us on 0844 477 3744, or contact us by clicking here.
Please fill in the form below and submit it to us. We will contact you to discuss your requirements.
Your pharmacist will then arrange a delivery time suitable for you. Please note - at least 48 hours notice is required for this service.
WE DO NOT TAKE PRESCRIPTION REQUESTS OVER THE PHONE If you are prescribed a new medication from somewhere other than the surgery you will need to provide details (you will not be able to do this by email or fax).
